Nurse with a familyThe RI Practice Transformation Network is a network of clinicians, practice managers, local stakeholders from all facets of the health care system and RIQI trained staff – a collaborative network of stakeholders working to transform the health care delivery system for optimal patient care in a value-based environment.

Transforming Clinical Practice Initiative (TCPI)

The Centers for Medicare & Medicaid Services (CMS) awarded RIQI an $8.3 million grant to set up a practice transformation network through its Transforming Clinical Practice Initiative (TCPI). TCPI is an opportunity for clinicians to improve patient health outcomes and quality of care while preparing for success under alternative payment models such as accountable care organizations (ACOs) that tie reimbursement to value and not volume. Eligible practices receive assistance at no cost.

Patient Centered Medical Home (PCMH) Assistance

RIQI can help your practice apply for and achieve National Committee for Quality Assurance (NCQA) PCMH recognition. We offer education and training, readiness assessments, on-site facilitation, technical support, and other tools and resources. RIQI is an NCQA Quality Partner and our Relationship Managers are NCQA Content Expert certified (CEC).
