CurrentCare For MeCurrentCare for Me collects your medical information from primary care providers, specialists, hospitals and lab/imaging facilities, and puts it in one safe place – for YOU! It’s different from your doctor’s portal where you may be able to send emails to staff or request appointments. CurrentCare for Me gives you the ability to:

  • Look up lab test results such as blood work, x-rays and MRIs
  • Track and double-check your medications
  • Download and send your records to caregivers
  • Assign Designees to help coordinate your care and receive alerts if you have an emergency

Designee ServicesWhat is a Designee?

A designee is someone you choose (spouse, partner, adult child, etc.) to have access to your CurrentCare for Me record to help coordinate your care when you need support managing a new health condition, treatment or medications. There are two access levels you can assign to your designee:

  • A designee with Full Access can view your CurrentCare for Me record, make edits to your account and receive Designee Alerts.
  • A designee with Alerts Only Access can receive Designee Alerts, but cannot view your CurrentCare for Me record or make edits to your account.

What are Designee Alerts?

Designee Alerts allow designees to receive email and/or text notifications when loved ones are admitted to or discharged from participating hospitals and skilled nursing facilities.

You can assign multiple people (at least 18 years old) to receive your CurrentCare Designee Alerts and/or have access to your CurrentCare for Me record.

Enroll by Mail

Fill out the Enrollment Form and follow the instructions below. Be sure to complete all sections, sign and return the form in one of the following ways:

  • Have your doctor verify your identify and submit the form for you.
  • Have your signature witnessed by a notary public and mail the form to:

  • Bring the completed form to RIQI for verification. Be sure to bring your government issued photo ID (due to COVID-19, we are currently working remotely).

 

How to Add a Designee

To add a designee online:

  • Log into your CurrentCare for Me Account
  • Click My Account
  • Click Manage Designees
  • Scroll then click Add a Designee

Follow the prompts to enter ‘Designee Type’ and your designee’s contact information. An email will then be sent to your designee.

Check the Terms of Service box, then click Submit.
